Pre-Season Sports Physicals
for
Middle and High School Athletes
As part of the Center Foundations Sports Medicine Program, free pre-season sports physicals are offered each August. This annual service offers middle and high school student athletes a no-cost way to obtain the required sports physicals. Thanks to the donated time and services of physicians, other medical professionals and volunteers, over 5,000 free student physicals have been conducted since the beginning of this community service.
Boys physicals: Tuesday, August 10, 2010 at The Center
Girls physicals: Thursday, August 12, 2010 at The Center
Physicals are offered on a first-come, first-served basis. Registration begins at 5:00pm and physicals begin at 5:30pm. Students must be at The Center no later than 6:45pm to receive a physical. A donation of $10 is appreciated. Signed OSAA parental consent form required
Who Is Eligible? Free sports physicals are available to all 6th and 7th grade middle school students prior to participating in their first athletic season and to high school student athletes prior to participating in their first athletic season and those students entering 9th and 11th grades
Additional Services
Youth Health Fair - Community organizations will also host a youth Health and Wellness fair with a variety of information related to healthy kids and families, interactive displays, raffles and refreshments.
New for 2010 - Injury Prevention Study
- Student athletes receiving physicals are invited to participate in a new clinical study to evaluate the risk of knee and leg injury. The Center Foundation understands that injury prevention is the best way to keep kids active and healthy. We are hopeful that the results of the study will help us identify athletes at higher risk for injury and provide targeted prevention measures to reduce injury risk.
Participation in the study only takes a few minutes and requires a signed consent form in addition to the physical forms required to complete your pre participation sports physical.

Immunizations -
Deschutes County Health Services will be offering Tdap, HPV and Meningococcal v
accines.
Please bring a your childs immunization record. If you have insurance (including OHP), please bring your card. If no insurance, there will be no charge for vaccines.
Parental consent required.
Important Note:
New Sports Physical Consent Form for 2010
Students are required to bring the necessary form WITH a parent signature. The student medical history and parent signature MUST be completed before the exam.
